Volunteer - State Mandated Forms and Information

Important!
Beginning July 1, 2015, an adult applying for an unpaid position as a volunteer responsible for the welfare of a child or having direct contact with children will need clearances.

If you are a returning coach or volunteer, you will need to complete the returning volunteer form - 2018 - Returning Volunteer.

If you are a new volunteer to our organization, you will need to complete a new volunteer form - New Volunteer

Please fill out the appropriate forms then return to your coordinator.   Electronic copies may also be submitted to Yvonne Teed at yvonneteed@gmail.com

If you are new volunteer to our league please also read further below as additional forms are required.

NEW VOLUNTEER INFORMATION
State mandated background checks: In addition to the annual Little League mandatory background checks for all volunteers, the Pennsylvania State Legislature has a law that all persons having repetitive contact with children have the following clearances, criminal history from the Pennsylvania State Police AND Child Abuse History Clearance from the Department of Human Services.

Additionally, a fingerprint based federal criminal history (FBI) submitted through the Pennsylvania State Police or its authorized agent is required if the position the volunteer is applying for is an unpaid position; and the volunteer has lived outside the Commonwealth of Pennsylvania in the last 10 years.
